Once your session is scheduled, you’ll receive a link to the secure client portal where you can complete the intake forms. These include demographic information, insurance details, and a brief questionnaire to help me understand your needs.
Completing these forms before our session allows us to make the most of our time together. If you have any difficulty accessing the portal or prefer paper forms, just let me know—I’ll be happy to provide an alternative.
I am in network with Blue Cross Blue Shield PPO and Blue Choice.
If I am out-of-network for your insurance, I can provide a monthly superbill (receipt) upon request. You may submit this to your insurance for possible reimbursement. Please contact your insurance provider to understand your out-of-network benefits.
Payment is due at the time of service.
I accept cash, check, all major credit cards, and HSA/FSA cards as forms of payment. Once your client portal is established, payment can also be made securely online through the portal.
If you’re unable to attend a session, please let me know as soon as you can. While I ask for at least 24 hours’ notice, I understand that unexpected situations arise. Late cancellations may be subject to a fee, but I do my best to be understanding when emergencies or unforeseen circumstances come up.
